To register for the instructional program, here's what you need to do:

• Print and complete "Instructional Registration Form"
  

• Get 2 copies of your child's birth certificate
  

• Register online at USA Hockey ($35.00 fee)
  

• $150.00 payment (checks made out to MCHA)
  

• Mail all of the above to:

Each registered player receives:

We are know accepting registrations for the Mt. Clemens Hockey Instructional Program!   We offer a "learn to play hockey" program that provides a 1.5 hour of instruction every Sunday to your son or daugther. You pay one price for the entire year!!!
